Employee Annual Notifications

Mount Shasta Union School District provides employees with the following annual notifications and requires a signed acknowledgment that such notifications were received and read.

Step 1: Scroll down and read each of the following documents.

Step 2: Print the Acknowledgment of Receipt, print your name, sign, date, and send to the District Office to acknowledge receipt of the above notifications. This document will be placed in your personnel file.
